The Health and Social Care Select Committee launched our inquiry into patient safety, performance and leadership in the NHS today with evidence from General Sir Gordon Messenger, who led a recent review on leadership and management in the NHS; Professor Rich Withnall, Chief Executive at Faculty of Medical Management and Leadership; Matthew Taylor, Chief Executive at NHS Confederation; and Rebecca Hilsenrath Chief Executive at Parliamentary and Health Service Ombudsman.

I focussed my questions on the action needed in the light of a recent NHS survey showing many staff were still reluctant to raise concerns about patient safety despite several initiatives to enable whistleblowing. You can watch my exchange with the witnesses below.
